Direct billing disclaimer

At Back to Motion, we strive to make the direct billing process as seamless as possible for our patients. However, please note that direct billing is a service we provide as a courtesy and is not guaranteed. The following terms apply:

  • Eligibility for direct billing may vary based on your insurance provider, plan specifics, and coverage details. A card on file is required at your first visit in order to direct bill.
  • It is the responsibility of the patient to verify their coverage, including any deductible, co-pay, or limits.
  • In the event of issues with claim submissions or if direct billing is not available, patients will be responsible for payment of services in full and will receive a receipt to submit to their insurer directly.
  • We are not responsible for denied or partially paid claims. Any unpaid balance will be charged to the card on file.

We appreciate your understanding and are here to assist with any questions about direct billing, questions regarding your insurance coverage limits or remaining balances should be directed to your insurance provider as we do not have access to this information.

Medical devices & product policy

At Back to Motion, certain products – including but not limited to compression garments, orthotics, and other medical devices – may be custom ordered or specially requested for individual patients.

Custom & special orders (non-returnable)
All medical devices and products that are:

  • specifically ordered for a patient
  • customized in size, fit, or prescription
  • not regularly stocked inventory

are final sale and not eligible for return or exchange once the order has been placed.

Patient responsibility & insurance claims
Patients are responsible for understanding their own insurance coverage prior to purchase.

  • Any estimates of coverage provided by the clinic are not guarantees of reimbursement
  • The patient remains fully responsible for payment of all products and services, regardless of insurance outcome
  • The clinic is not responsible for denied or partially paid claims

In cases where a claim has been submitted, the clinic may request confirmation of claim status or reimbursement to assist with account reconciliation.

Payment authorization
By proceeding with an order, patients acknowledge and authorize:

  • the purchase of the product(s)
  • payment for all associated costs
  • use of the credit card on file for any outstanding balances

Returns & exchanges (non-custom items only)
In limited cases where a return or exchange is approved for non-custom, unopened items, the following will apply:

  • A 20% restocking fee
  • A 5% merchant processing fee

All returns must be:

  • approved in advance
  • in original, unused condition
  • accompanied by proof of purchase

Final sale acknowledgement
By proceeding with a purchase, patients acknowledge and agree to the terms outlined above.
